The OPAT+ service supports the administration of IV antimicrobial therapy in the patient’s own home / care setting, or ambulatory setting.

It also supports patients with complex infections on oral regimens.

The aim is to provide gold standard care to our patients with complex infections in outpatient / home settings.

You can make an E-purple referral from both sites (BNHH & RHCH) to 'BNHH: Infection: OPAT' - Mon-Fri, 8.30am - 4:30pm

Please consider referring patients who are within 24-48hrs of being MFFD, have had all their investigations completed and have appropriate vascular access.

When communicating with patients please let them know it usually takes at least 48hrs to coordinate discharge as we need to ensure capacity in the appropriate district nursing/ambulatory team or teach the patient how to self-administer; additionally we need to ensure witnessed administration of the first dose of any regimen.

  • Patient has received first dose of anti-biotic on ward and has been tolerated.
  • Patient is ≥ 18 years of age
  • Patient is medically fit for discharge within 48 hours
  • Patient has no fever recorded in preceding 48 hours
  • Significant improvement in inflammatory markers (CRP, WCC, ESR etc.).
  • Patient is able to understand the rationale and treatment plan, is willing to have IV therapy at home and gives consent
  • Patient is not an active IV drug user
  • Patient is on a suitable / manageable drug regime
  • Patient has suitable social and domestic circumstances - consider:
    • Water / electricity
    • Mobility for access to visiting staff and deliveries
    • Mode of transport (in case of emergency)
    • Family support
    • Acceptable standard of hygiene
    • Telephone
    • Outpatient follow-up arranged with referring clinical (or documented plan)
    • Contact details of referring clinical provided
