The CHS Healthcare service is commissioned by HHFT (Winchester/Basingstoke/Andover – Kingfisher ward) to help support privately funding patients needing to source care once classed as fit for discharge.
We have been in post since 2016, and have helped countless patients and their families to source safe and appropriate care in an aim to discharge patients swiftly with our ‘hand holding’ support. We understand that many families can feel overwhelmed on what to do next or where to start when looking for care. CHS Healthcare have over 20 years’ experience and understand what questions to ask and what information is crucial for a successful discharge.
Within the local area we have hospital schemes, similar to HHFT, operating in Portsmouth QA Hospital, UHS Southampton Hospital, Royal Bournemouth Hospital, Wexham Park Hospital, Royal Surrey as well as supporting the community too. Therefore, we have extensive knowledge of the Hampshire county area and also peripheral counties.
The HHFT CHS team are based in the Integrated Discharge Bureau at Winchester Hospital, and the service is easy to access by contacting the number below. We ideally ask that a referral is provided to the team as per the attached document (this asks a series of questions that we can help with if you need any guidance in completing the form). We allocate an adviser per referral so that there is one point of contact for the patient and their next of kin.
