The Achieving Priorities of Care (APoC) document was developed in 2014 and is reviewed annually. It has been designed to guide care for people identified as being in their last days and hours of life, to ensure we achieve the national five priorities of care:

Recognise the possibility the person is likely to be dying

Communicate with person and those important to them

Involve them in decision-making about treatment and care

Support the needs of the person and those important to them

Plan and Do with an individual plan of care

The document ‘achieving priorities of care’ (APOC) replaces the normal medical and nursing notes when someone is thought to be dying. 

It guides doctors and nurses to ensure people receive all the aspects of care they require at this time, to allow them to be cared for in the best way for them and helps to ensure they are as comfortable as possible.

The document is stored electronically ( EPR - eForms) and care is recorded every 2 hours.
