Elderly Care Mission Statement 
The Elderly Care department at HHFT consists of a comprehensive service covering inpatient acute wards, discharge to assess (D2A) wards, an acute rehabilitation ward on the Andover site, an enhanced frailty MDT, and outpatient clinics across hospital & community across Basingstoke, Winchester, & Andover.
The service is supported by an administration team with members working on both the Basingstoke and Winchester sites. In addition, the service is operationally led by an operational service manager team.
The service consists of staff across multiple disciplines including:
Consultant Geriatricians
SPRs and Speciality Doctors
Consultant Nurses
Clinical Matrons
Clinical nurse specialists
Frailty pharmacist
Nursing and HCA staff
Specialty administration

The service has both clinical and operational leadership leads:

Operational Service Manager – Francesca English.
The primary purpose of the service is to support and enable patients who have 2 or more of the frailty syndromes listed across and score 6 or higher on the Rockwood score to live well with frailty.
