  1. The IHORM is the risk assessment form that must be completed 1st and if their level of risk is not too high then the patient must sign a home oxygen consent form which is at the back of the IHORM form. This is a legal document and must be kept in the patients notes and a copy sent to the community home oxygen team.
  2. The clinical code must be completed, otherwise HOOF will be rejected.
  3. Total hours of Oxygen delivery must add up to no more than 24hrs or else it will be rejected.
  4. Every patient who receives a concentrator is given a backup (large) cylinder.
  5. Prescriber should ensure that there is someone at the patients' property to receive home oxygen equipment, the oxygen company will not leave equipment if no-one is home.
  6. Try and avoid the 4hour urgent deliveries as they are expensive. 3 day and next day deliveries are encouraged where possible.
  7. No home oxygen for smokers, a patient is considered a non smoker if they have not smoked for 12 weeks. If they smoked prior to admission, they should be referred to the community home oxygen team for assessment at home before home oxygen is prescribed.
  8. Standard Ambulatory cylinders do not deliver 8 and 11 litres of oxygen. if you request 8 or 11l/m for ambulatory oxygen, HOOF will be rejected.
  9. Home oxygen can be delivered up to 15l/m.
  10. Completed HOOF must be emailed to hoof.dv@nhs.net.
  11. IHORM and referral for follow up must be sent to  respiratory-assessment@southernhealth.nhs.uk. If patient is from Berkshire follow up referral must be sent to rbftrespiratory.nurses@nhs.net
